Meaning

Better open enmity than the friendship of the muddled; better death than an incurable cruel disease. Kinder than scorning one who deserves pity is to kill him; and better than praise is abuse.

Explanation

The verse lays out four comparisons in a row, each harsher than the one before. The first is plain. Open enmity is better than the friendship of those who have no clarity. In enmity you can see where the blow is coming from; in a muddled friendship you cannot.

The severity then climbs: better to die than to live with a cruel disease that will not heal. The last two comparisons point to a single thought — contempt, and praise that carries no affection, are both deadly. Hence 'there is nothing else.' Praise without love is worse than abuse; the two are the two faces of contempt.

To reflect on

Better a known enemy than a friendship you cannot read.
