Explanation

The final couplet of the chapter divides treatment into four parts. Only when four are present, the patient, the physician, the medicine and the attendant who stays by, does the disease go. The "uzhai chelvaan" is the one who stays beside the patient and serves; Ayurveda calls him the parichaaraka. If any of these four is lacking, treatment is incomplete. If the patient refuses the medicine, what can the physician do? If the medicine is poor, the physician's knowledge is wasted. If the attendant is careless, the patient recovers slowly.

Valluvar adds that each of the four has its own qualities. In "appaal naarkoorre" the old texts say each part has four qualifications. The patient should want to be cured, follow the physician's advice with discipline, have the means to pay for medicine, and have a firm mind. The physician should have book-learning, experience, purity and compassion. This shows the depth of the medical ethic of those days.

In Hindu tradition Charaka calls these four the chatushpada. Valluvar's view is in harmony with it. The life lesson is that any good work needs the contribution of many. One alone cannot succeed. Health is born only when the patient, physician, medicine and attendant come together.

We see the same in hospitals today. Even with an excellent physician, if the patient's kin do not attend responsibly, recovery is delayed. The patience and love of the attendant often become a better medicine than the medicine itself. So Valluvar honours all four equally; none is to be thought lesser.

A story

Sundaram's Illness and the Four

This is an imagined tale; its setting is a Tamil village family.

In a village near Tiruvaiyaru lived Sundaram, a nadaswaram vidwan. A severe fever afflicted him. Many days passed without relief. His wife Chellamma took him to Ramanathan, a famous vaidya of the town. The vaidya felt the pulse and said, "This is no ordinary illness. It needs long care. I will give the medicine, but both of you must cooperate."

The vaidya explained the four: "The patient is Sundaram, the physician is I, the medicine is this herbal decoction, and the attendant is your wife Chellamma. If these four come together the illness will go." At first Sundaram hated the bitter decoction. "I cannot drink this," he said with a grimace. The vaidya said, "If you refuse, my knowledge is of no use." Sundaram changed his mind and drank the decoction with full resolve.

Chellamma stayed beside her husband day and night. She gave the decoction at the right time. She cooked simple gruel and fed him. She refused the spicy food he asked for. She even received her husband's anger with a smile. The vaidya came every day, felt the pulse and adjusted the dose.

In three weeks Sundaram was completely well. When he thanked the vaidya, he said, "Not to me alone; thanks are due to all four of us, especially to your wife." On the day of the Tyagaraja Aradhana at Tiruvaiyaru, when Sundaram played the nadaswaram on the stage, he dedicated the first music to the vaidya and his wife.

To reflect on

In a task that matters to you now, which of the "four parts" is missing: the one in need, the one who knows, the right means, or the one who stays by?
